How it works:

CCM Care Managers

The CCM Care Manager’s role is to identify triggers or problems so that they are communicated to the physician in a timely manner. Communication between the Primary Care Practitioner, Specialists, and all members of the Care Team is improved.

Monthly communication allows physicians to maintain control over the healthcare plan while our team facilitates initiatives to:

  • Monitor treatment goals

  • Manage symptoms

  • Plan interventions

  • Manage and reconcile medication

  • Coordinate with outside resources and providers

  • Update the patient’s care plan when applicable

Ensure all MIPS-related categories of care are addressed, assuring that the practice meets MIPS high annual point requirements.

Working as an extension of the practice, we can address any issues affecting the patient’s life. The Care Manager's greatest goal is to achieve the highest level of wellness for the patient while reducing chronic disease and hospitalizations.
